Free and Reduced applications are available via the board office or print the form from your home computer and return to the Madison Board Office. Filling out these forms ensures your student(s) will have free or reduced price meals on the first day of school. You must use the current school year form. Parents are responsible for their students lunches until they receive notification that the application has been processed. Click HERE to download this year's form.
The Madison Local School District strives to buy American-grown foods whenever possible. Buying American foods for our school district helps to ensure high-quality meals for our students, and supports our country's agriculture industry. Ninety eight percent of our schools food is raised in the United States, or is from our local agricultural industry. The only foods, currently served at Madison Local Schools that are not raised in the United States, include bananas and mandarin oranges.

The Madison Food Service and Child Nutrition Program is committed to providing nutritional meals to all of the district's students. Madison Schools participates in the National School Lunch Program and are required to meet one-third of a student's daily caloric needs. Madison Schools follows the Offer versus Serve policy sponsored by the USDA. Offer versus Serve is intended to decrease waste and costs by allowing students to refuse up to two components in a lunch and one component in a breakfast. The lunch components consist of an entree, vegetable, fruit and milk. The breakfast components consist of an entree, fruit/juice and milk.
We serve breakfast in all of our schools before the school day begins. We hope your students take advantage of this important meal because research has shown that students perform better in school; have a better attendance and score higher on tests if they have had breakfast. Our breakfast serving times are as follows:
All Head Cooks are ServSafeCertified. This certificate is accredited by the American National Standards Institute - Conference for Food Protection and the National Restaurant Association Educational Foundation.
